Lost Debit Card

To Report a Lost or Stolen ATM/Debit Card:

1. Call CPS at 1-888-297-3416 to report the card lost or stolen.
2. Call First Bank at 319-352-1340 or 319-242-7151 to cancel your card. If the bank is closed, please leave your name and phone number and a bank representative will return your call during business hours.
3. Verify your account transactions with a First Bank Representative.
4. A bank representative will process an application for a replacement card.
